The National Grange is raising the alarm on a growing issue within the Medicare 340B program. A loophole known as “dual-classification” is allowing urban hospitals to access resources intended to support rural providers.

The result is real and visible in communities across the country. Rural hospitals are closing. Access to specialized care is becoming more limited. At the same time, federal spending continues to rise, but not in ways that strengthen the rural healthcare system it was meant to support.

Closing this loophole is about restoring balance and ensuring that critical resources reach the rural hospitals and patients who depend on them most.

The 2026 National Grange Leaders' Conference is in full swing. With about 70 leaders in person and more joining us virtually, we kicked off yesterday in Sparks, Nevada, with presentations and workshops that will inspire Grange leaders to embrace our theme, "Together We Grow," with a vision for the next 5, 10, 25 years, and the incredible capacity and strength that Grangers have.
This morning, National Grange President Christine Hamp gave her keynote address about picking up speed and stepping out of our comfort zones to be the leaders the Grange needs as we embrace the future.

Show the Work emphasizes the importance of demonstrating progress, transparency, and follow-through. It means making visible the effort, planning, and results behind decisions and initiatives - not for recognition, but for trust-building and accountability. Show the Work reinforces that credibility is earned by action, not intention. Within the Grange, this concept reminds leaders and members to communicate steps taken, milestones reached, and outcomes achieved so that momentum is shared, understood, and celebrated across all levels of the organization.
